Somatic Grief Therapy: Release the Weight of Loss

When you experience a devastating loss, your whole world changes instantly. Friends and family might check in on your emotional state, and may focus on how your mind is coping. They may not realize, and you may not either, that the physical toll of grief is just as heavy as the emotional burden.
A Grieving Body, as Dr. Mary-Frances O'Connor writes in her 2025 book, is adjusting to a horrible shock. It's only natural that you will sense changes in your physical body when you go through this. For instance, you might find yourself struggling to get out of bed. Your limbs might feel like lead. Your chest aches, your jaw feels tight, and you are entirely exhausted despite sleeping for hours. Alternatively, you might feel high anxiety and be unable to sleep very well.

Body-based therapy (aka somatic grief therapy) can help re-regulate your nervous system and attend to these physical changes.
The Somatic Architecture of Grief
Cumulative stress and profound loss do not just live in your thoughts. They get locked directly into your nervous system. When a significant loss occurs, your brain interprets the emotional severing of a connection as a threat to your actual survival. This registers in the body as danger, instantly throwing your autonomic nervous system into a chronic state of fight, flight, or freeze.
When your nervous system is stuck in survival mode, it diverts energy away from resting, digesting, and healing. Grief doesn't just affect the nervous system, however. It also impacts the cardiovascular system, which is more taxing on your heart. Your hormones are disrupted, and your immune system is compromised. The result of all these physiological changes is a series of very real, very heavy physical symptoms:
• Systemic Exhaustion: Your body is running a continuous background program of high-alert stress, burning through your physical energy reserves.
• The "Grief Ache": Chronic muscle tension, particularly tight shoulders, a clenched jaw, or a heavy sensation in the chest.
• Brain Fog and Sluggishness: Because your nervous system is overwhelmed, processing daily information feels incredibly slow and frustrating. Your short term memory might be poor, and your concentration may slip significantly.
• Digestive and Immune Drops: A direct side effect of the body staying trapped in a survival loop instead of a restful state. It's easier to get sick, and you might feel less hungry. It's important to remember to eat regularly, even if you don't feel like having a healthy meal.
Why Traditional Talk Therapy Can Leave Healing from Grief Incomplete
Many people try to think or talk their way out of physical grief. You might tell yourself, "It has been months, I should have more energy by now," or analyze your loss repeatedly in conversation. Putting pressure on yourself to complete the process prematurely only makes you feel shameful and guilty. You get to grieve for however long you need to. You're not on anyone else's timeline.
While sharing your story is valuable, traditional talk-only therapy can sometimes feel like it only goes so far. Talking about the loss can be helpful if you don't get emotionally flooded, but it only addresses your cognitive processing of the loss. You need to integrate the emotional, physical, and spiritual aspects as well.
For lasting change, you need interventions that send a safety signal to your hypervigilant brainstem and nervous system. To genuinely let go of the physical weight of loss, you have to bypass the overthinking mind and work directly with the body's subconscious inner resources.
